Program Description
Growing Up With Hope provides family-centered Case Management services with individualized support services. Families are offered referrals to receive support to complete education, secure employment, strengthen parenting skills, and transition to permanent housing.
Participants engage in goal planning with case managers and receive referrals to childcare, healthcare, mental health, and substance use and alcohol use dependency services. Families receive individualized birth planning and in-home postpartum care through partnerships with clinics, hospitals, and harm reduction programs, supporting parental capacity, recovery, and the safety and well-being of both parent and child.

Statistics in the US and Western Massachusetts
About 3 in 10 teen girls in the U.S. become pregnant at least once before age 20. Teen parents are significantly more likely to live below the federal poverty line. Children of teen parents face higher risks of housing instability and school disruption.
•Western Massachusetts counties had teen birth rates 30–98% higher than the Massachusetts average (except Hampshire County).
•Hampden County teen birth rate: 16.8 per 1,000 teens (ages 15–19).
•Among Latinx teens in Hampden County: 39.8 per 1,000.
•Latinx teens accounted for 74.4% of teen births in Western Massachusetts.
